Get Form T2201 Download from CRA website or request from your doctor's office. Part A is filled by you, Part B by your medical practitioner.
Book appointment with your doctor Your doctor, psychologist, or occupational therapist must complete Part B. Explain how your condition markedly restricts one or more basic activities of daily living, or significantly restricts two or more cumulatively.
Submit T2201 to CRA Mail or submit electronically through CRA My Account. Processing takes 2-8 weeks. Once approved, claim DTC on current and prior year returns.
If approved: file T1-ADJ for prior years You can claim the DTC for every year your condition existed, up to 10 years back. Each year = ~$1,965 refund.
